Waiver of Liability & Parent Consent

Event: Christian Youth Nights

Date: Saturdays once a month, starting as of Nov. 2, 2024

Location: Davie FL 33325

As the parent or legal guardian of the child in the RSVP, I acknowledge that I am voluntarily allowing my child to attend the Christian Youth Night hosted at the specified location. I understand that participation in any activity may involve certain risks, including but not limited to physical injury, illness, or other unforeseen events.

Release of Liability:

I, the undersigned, hereby release, waive, discharge, and covenant not to sue The House Church, We are Church, including any of its churches, its directors, officers, volunteers, and/or agents from any and all liability for any injury or damages sustained by my child while participating in this event. This release applies to all claims, whether known or unknown, arising out of or in any way connected with my child's participation in this event.

Medical Emergency:

In the event of a medical emergency, I give permission for my child to receive necessary medical treatment. I understand that every effort will be made to contact me as soon as possible.

Photography Release:

I grant permission for photographs or videos taken during the event to be used for promotional purposes by The House Church, including social media and printed materials.

Acknowledgment:

By agreeing to the terms, I confirm that I have read and understood this waiver. I am aware that I am waiving my rights and that of my child.
